Fire safety inspection before use or opening of public gathering places

1. Information required by the construction unit

1. One copy of the business license of the construction unit;

2. One copy of the ID card of the legal person of the construction unit;

3. One copy of the authorized agent’s ID card;

4. A copy of the original design review opinion of the building where it is located, a copy of the building fire protection acceptance opinion, or a copy of the legal document of the fire protection record obtained according to law;

5. One copy of the vocational qualification certificate for the special type of work in the fire protection industry obtained by the operator of the automatic fire fighting system on duty.

2. Information required by the design unit

1. Two copies of the unit's business license with official seal;

2. One copy of the unit’s legal person’s ID card with official seal;

3. Two copies of the photocopy of the qualification certificate with official seal;

4. A copy of the designer’s qualification certificate and ID card with official seal in one copy;

5. The designer's information registration form (detailed attachment 1)

6, a full set of design blueprint cover design drawings in 2 copies

4. A copy of the construction management personnel qualification certificate and ID card with official seal in one copy;

5. The registration form of construction management personnel information shall be stamped with the official seal (detailed in Annex 1);

6. The design blueprint is stamped with the completion seal of the construction unit in 2 copies;

7. One copy of the fireproof performance test report and qualification certificate of decoration materials during the construction process;
