1. Information required by the construction unit

1. Two copies of the business license of the construction unit;

2. Two copies of the ID card of the legal person of the construction unit;

3. Two copies of the ID card of the authorized agent;

4. One copy of the fire protection design review opinions;

5. The original version of the lease contract and the agreement for decoration on the official website of the Bureau of Housing and Urban-rural Development (with the seal of the lessor and the user). 2. Information required by the design unit

1. Two copies of the unit's business license with official seal;

2. One copy of the unit’s legal person’s ID card with official seal;

3. Two copies of the photocopy of the qualification certificate with official seal;

4. A copy of the designer’s qualification certificate and ID card with official seal in one copy;

5. The designer's information registration form (detailed attachment 1)

6. The design blueprint that has passed the plan review is stamped with the completion seal in 2 copies;

7. One copy of the fireproof performance test report and qualification certificate of decoration materials during the construction process;

8. One copy of inspection report for fire protection facilities.
